You’re often the last stop. The patient who reacts to every commercial version of a drug lands at your counter. The referral note says “sensitive to fillers, please compound.”

Which fillers? Nobody upstream could say. ExciIQ turns that guess into a workup.

Sort the patient’s medication history into tolerated Friends and reaction-associated Foes. ExciIQ calls this workup Friend or Foe.

ExciIQ lists the suspect excipients that appear only in the Foes. When a suspect appears in every Foe, it is flagged as a Top Suspect.

Check each suspect against the full regimen, not one label at a time. ExciIQ shows cumulative daily intake per excipient across everything the patient takes.

ExciIQ checks commercial equivalents against the trigger and gives each one of three honest verdicts: safe, contains the avoided excipient, or uncertain. A verdict of safe means the product provably avoids the trigger. Uncertain is never dressed up as safe.

For the retail / community pharmacist

The refill matches in the system: same drug, same strength. But your wholesaler switched manufacturers. Three weeks later the patient is back describing a rash.

The active ingredient is unchanged. The inactive ingredients are a different story, and until now an invisible one.

For the allergist, immunologist, or physician

The chart says “multiple drug allergies.” The patient says the reactions never quite made sense: different drugs, different classes, the same rash.

When the actives don’t explain the pattern, the excipients might. The same dye, binder, or preservative can thread through otherwise unrelated products. It can enter or leave the picture with a manufacturer switch the patient never knew about.

ExciIQ lets you work that hypothesis with the real products in hand. Pull each product the patient received by name, NDC, or pill imprint. See its inactive ingredients as labeled, specific to manufacturer and strength.

Sort what was tolerated from what drew a reaction. ExciIQ shows the candidate excipients unique to the reaction-associated products. That is a shortlist to carry into your workup, not a verdict.

Then work that shortlist forward. Look up any excipient to see which products contain it. Find equivalents that provably avoid it for the note back to the prescribing team and the pharmacy.

Every record carries provenance. When a label disagrees with itself, you see it. A workup is only as good as the ingredient list under it.

The formulary didn’t change. The supplier did.

Shortages and contract switches swap one manufacturer’s product for another’s. The inactive ingredients swap silently with them.

For most patients it never matters. For the patient with a documented sensitivity to lactose, a dye, or a preservative, “same drug, same strength” isn’t the whole answer. A check aimed at active ingredients won’t see it.

Veterinary

The patient who can’t tell you it’s the excipient.

Animals metabolize differently. The excipient that’s harmless in your hand can be the problem in the exam room. Xylitol, safe in a human tablet, is dangerous to dogs.

Veterinary practice lives with this every day. Human-labeled products are dispensed under extra-label use.

Species sensitivities appear that the label never anticipated. Compounding requests are built around what an animal can’t tolerate.

ExciIQ covers FDA’s human labeling today, prescription and OTC, at product-and-strength precision. That is the label set extra-label dispensing draws on. We plan to load FDA’s animal labeling in Q4 2026.
